The Community First Choice program allows individuals who qualify for Medicare due to disability to receive permanent or temporary in-home care services under Maryland state law. Since its establishment in 2010, this policy has helped millions of Americans receive community-based assistance. Under this policy, you or a loved one may be able to secure an in-home Personal Care Assistant at no cost to you.
After the enactment of this policy, many people were able to seek care who were unable to secure it before - though this presented a new problem: what is the best way to ensure that each individual is matched with a provider that best suits their needs?
To answer this question, Community Health and Education Services LLC was founded. Their mission is to help you and others in your situation get much-needed care under the CFC program, and to do so, they can provide assistance every step of the way.
You can now contact CHES to be connected with one of the many certified RNs, LPNs, and in-home care specialists in the company’s professional network. These providers are covered under the DDA medicare waiver and have been hand-selected by the team at CHES for their commitment to quality care.

We provide services in your home or the place you call home. If needed, we also connect you with appropriate community support services.”
To request assistance, you or a loved one may reach out to CHES directly, or in the form of a referral. In either case, the organization will submit a request for a DHS agent to be dispatched, who will then make a determination regarding a given patient’s qualification status for a Personal Care Assistant.
