Community Health and Education Services’ (CHES’s) support broker services will facilitate communication and coordination between you, your loved one, and their service providers, such as personal care assistants (PCAs), registered nurses, and DDA representatives.
If your loved one has opted into a self-direction program, CHES staff will work with both of you and your primary care provider to create a customized care plan that best meets your loved one's needs and goals. Agency professionals are trained to assist clients with cerebral palsy, spina bifida, and those on the autism spectrum.
Besides creating care plans, support brokers can assist you and your loved one with managing their budget and with navigating and accessing available resources. Through CHES, these resources include skilled nursing, personal care, and Developmental Disability Agency (DDA) nursing support services.
If your loved one participates in the DDA’s self-direction program, through CHES they can take advantage of nursing consultations or nursing case management and delegation services. The latter services are required if you or another family member is considered paid staff who administers medications or treatments.
